 Instructions for Lien Request Form 

After you have read the instructions below, click above to fill out the form.  The form you will complete will open in a separate window for easy access to these instructions.

  • Your Information
    Do not include comments or explanations within your answers to any of these questions. At the end of this Lien Request Form, there is an area for such information. For this section, be sure to use full legal names. You must have an e-mail address as all communications regarding your Lien Request Form will be via e-mail. For the title of person who will be signing the documents, some examples for a corporation are: President, Vice President, Secretary, or Director. For a limited liability company, some examples are: Managing Member or Member.


  • Your Customer's Information
    This is the person or entity who hired you. If you are unsure of an entity's name, go to www.sunbiz.org. If you are unsure if your customer is the owner of the property, refer to the Notice of Commencement, your Notice to Owner, or the property appraiser's website for the county where the project is located. If your customer is not the owner of the property, you may need to make some phone calls or conduct on-line research to obtain information regarding ownership of the property and what parties are involved with the project.


  • Property Owner Information (as Listed on the Notice of Commencement)
    You should have the Notice of Commencement in front of you in order to answer these questions. If you do not have the Notice of Commencement, the Official Records Book and Page Number where it is recorded on the public records should be specified in your Notice to Owner. You may need to make some phone calls or conduct on-line research to obtain a copy of the Notice of Commencement. If you used NACM Tampa or NTO Management to prepare your Notice to Owner, all you need to do is provide your Log # or Reference # and we will obtain the necessary information.


  • Notice Information
    You should have your Notice to Owner in front of you in order to answer these questions. The owner the owner's address and the date it was sent should be listed at the top. Those who received copies should be listed at the bottom. The method of delivery as well as the certified mail, registered mail, or tracking number should be listed above or next to the addressees' names.


  • Notice of Commencement (Property Owner Information and Property Information)
    You should have the Notice of Commencement in front of you in order to answer these questions. If you do not have the Notice of Commencement, the Official Records Book and Page Number where it is recorded on the public records should be specified in your Notice to Owner. You may need to make some phone calls or conduct on-line research to obtain a copy of the Notice of Commencement. If you used NACM Tampa or NTO Management to prepare your Notice to Owner, all you need to do is provide your Log # or Reference # and we will obtain the necessary information.


  • Transaction Information
    A description of the labor, services, and materials you furnished should be set forth in your contract, invoices, purchase orders, or Notice to Owner. It is important that your answers provide accurate dates and amounts because this will be listed in your Claim of Lien and affect the validity of your lien and your exposure to recording a fraudulent lien. With regard to any amounts you owe to subcontractors and suppliers, even if you believe you were not hired by the owner of the property, we will check to see if there is a common identity between your customer and the owner and, if there is, we will need this information in order to prepare a Contractor's Final Payment Affidavit.


  • Additional Comments and/or Explanations
    Do not include comments or explanations within your answers to any of the above questions. This area is for such additional information.


  • What happens after I hit "SUBMIT"?
    Within 1 business day, FCL will send you an e-mail furnishing you with a link to make payment and specifying which of the 4 lien packages is required based upon your responses in the Lien Request Form. Upon receipt of payment, FCL will send you a confirming e-mail that the appropriate fee and your properly completed lien request have been received. Said confirming e-mail should be sent within 1 business day after your fee is received by FCL. That e-mail marks the completion of your lien request. For "rush" packages, the applicable documents will be completed within 72 hours of the completion of your lien request. For "non-rush" packages, the applicable documents will be completed within 7 days of the completion of your lien request.
